Rating should never take the place of parental involvement/choice. Agree that a lower age range in the survey makes sense.
i would even suggest lower - the results could be very interesting.

(Follow-up, after looking the survey over again.)
I just re-read the survey. My option was the first listed one in the ethnicity question, so I never originally looked through the others. The choices are a bit strange, and technically only contain two ethnicities. I could see being a bit weirded out by the question, in restrospect.

Done, although I would've liked the option 18< in question 3. I'm 16, so I would've preferred an option like that.

I would make a version with less open ended questions so it's easier to interpret your data if you want alot of people to reply to it.

Again, I am not the one that made the Survey.
She is doing it for a project, so most likely the Questions reflect what is required of it.
I can ask her to put the results, but it is up to her. I will try to sway her when she is in a good mood.
