I didn't ever leave a form for battle.

This...plus using mouse form for scouting in stealth, to be able to peek around a corner and get targeting info without alerting the mobs.Creature 1 wrote...
Burning man -> fireball. Spirit form -> crushing prison. Golem form -> quake and slam.
The Fade is easy.

I have come across a few battles where those darkspawn can swarm you fairly quickly and they don't miss. Its weird. They always hit my character and he has high dex. I also have fairly low HP so even when I go in Golem or any of the other modes, I still have low HP. I will get through it, but I am on hard and I know I will face a few difficult spots.
If you're letting the bad guys get in to short range alive, you're possibly doing something wrong. Plus, cycle through different forms to regenerate health...

If you get swarmed, burningman's fireball on top of yourself works well as does the golem's quake. The fireball does damage, DoT, plus knocks them down away from you. While quake usually stuns them.
Actually, if you are about to be swarmed and can't head it off, I like to place a fireball right in front of me. That way when I get knocked down, it's in the opposite direction and gives me more time to handle things.
