ME1 - Antibaar Geth outpost.
Those scavengers you run into on the east side of the map doing Major Kyles mission can be tough when you're low level early game.
ME2 - Object Rho.
The two Ymir mechs on Sanctum.
ME3 - Piece of cake. The only really hard bit on Insanity is the Mirror Match in the Arena.
The Atrium on Grissom Academy can be a pain though.
ME1 - Fully agree with Rane above me; Antibaar Geth outpost is really frustrating to deal with.
ME2 - Horizon final area. With 2 scions, a bunch of husks, collectors, harbinger collector, and the pretorian (not to mention the areas of cover aren't that great in this area) makes this mission the hardest one for me in the game.
ME3 - I guess the last mission/Maruader shields can be a pain in the @$$ on insanity (no joke!)
For me, it probably has to be:
1 ) Horizon (the last section with the Praetorian and Scions and a mission husks and Collectors)
2 ) Grunt's loyalty mission (the last section with the bunch of Krogan, YMIR Mech, and rocket-spamming Jedore)
Both on Insanity
Closing those F****** shutters in Garrus's recruitment mission. (Not with a new game plus) Imagine you're on insanity with a level 4 or so infiltrator. You got a shuriken, a predator, and a mantis vs (seemingly) unending krogan, vorcha, and varren with ever-trusty Jacob at your side...
