Do I change the options at the main menu first, before creating the character?

I'm assuming if I switch to Insanity at the start in Vancouver, I'm not going to get credit for it.

As the Xbox achievment description says

"Finish the game on Insanity without changing difficulty after leaving Earth."

That means you can have it on whatever level you like at the start while in Vancouver

Yeah you must switch to insanity before Vancouver ends.

Don't worry, as long as you turn it to insanity before leaving earth you'll be fine. And insanity on ME3 is preaty much a cakewalk (depending on what level and class character you have). I saw little to no difference between it and ME2 on hardcore...maybe veteren :P

...Ah... well fooy.

I blasted through Vancouver/Hardcore with my engineer so fast I forgot I had meant to do an Insanity run, upped it two steps off the shuttle to Mars and was hoping that would do it. Oh well, at least I'll get the other Insanity achievement . (Unwavering specifically, never run a Gold MP)
