 If I acquired and chose intel upgrades, what happens to them in the a new game plus?
Are they perminant, are they reset, can I reset them myself somehow, or do I have to reacquire them?

They reset and the offer to get them again will come up from glyph on your next playthrough. Its not set in stone so you can easily see what you like the first time and re choose stuff or pick something different

I checked a game of a prior play through, I have upgrades in the intel, about 5 selected. In the game after that, new game plus, my earliest save I have 1 intel upgrade option set, nothing else. How do you choose them form glyph? I dont see a option to get it from him, and the terminal doesnt have my past options from the prior game.

You have to reacquire them. the only thing that is set is the scar removal. You'll keep this, and when you re-earn the intel that gave it, you can only select the health bonus.

So they must be reacquired, but when you get the scar healing thing again, you can get health only. Were the options health or heal scars the first time? Or is health just all you get in place of scar healing?
I really didnt pay much attention to this during my ME3 play throughs..

Health or heal scars yeah. And I think if you healed the scars in ME2 then you only get health in your first ME3 playthrough as well.
