Is it possible to kill/get rid of all companions?

 So I know you can kill/ get rid of some companions so they are no longer avaliable in the party:

Isabella - don't recruit/give to Arishok
Fenris - give to Danarius/don't recruity
Bethany/Carver - die in prologue/deep roads/ kill Circle Mage Bethany on Templar side
Anders - MURDER KNIFE
Sebastian - keep Anders alive
Aveline - something about not doing her sidequests? does anyone have any more information on this one?
Merill - not sure
Varric - obviously none

Does anyone have any information on other ways to kill/get rid of companions? It's always good to have the information for an Evil! or Fail! Hawke XD

Especially Merill, because I can't remember anyway for her, and she's a compulsory companion. For instance, if I dont raise her rivalry or friendship enough will she leave like I've heard Aveline does?

Not doing Aveline's personal quest ruins her chances with Donnic (so no marriage), but does not necessarily get her to leave. However, Aveline is one of the pro-templar companions: if you side with the mages, she'll leave the party unless you've done her quests and reached 100 Friendship/Rivalry. But she won't attack you, like some others.

For Merrill: According to the wiki, Merrill will side against Hawke at the end if you choose to side with templars, did not complete all her quests, and did make Anders a 100% Rival and did his personal quests.

There's also the other "arg you sided with X at the end" companions, Fenris and Anders.

EDIT: Oh! Isabela! She'll abandon you if you don't get her to 50 Friendship/Rivalry by the end of Act II.

If Merrill is not at 100% friendship or rivalry before the last straw and Hawke sides with the Templars. Merrill will leave you. If Merrill is a rival to Hawke and not at 100% rivalry, then you do have to fight her.At least thats what the DA2 guidebook has to say on the matter, never tried myself.

Ah, so no other way than in the final battle then? And none if you side with the mages?

Anders can be told to sod off and will leave the party during Act II, after the "Dissent" quest (the one with Alric and where Anders kills or almost kills a mage girl).

Do anything pro mage and Merrill by virtue of being a mage herself will stick by your side.You will get the opposite of what you are trying to achieve. So in answer to your two questions, no.

But what about if she's never present in the party and her sidequests arent'd one and so never gains friendship or rivalry points? Does she stick around even if she stays at 0 points?

She has no real reason to leave Hawke's side if you side with the mages, even if you haven't helped her at all. At least, not exactly at that point. Even if Hawke ceased to exist at that exact moment, she'd probably go and help the mages anyway.

She will leave you during the epilogue, though, but every companion does that except if they're romanced.
