Sorry, saw this question and thought it was a bit silly.
Clearly Shepard's LI hesitated because they didn't want to think that Shepard had died.
Taking into account what Shepard has been through in the past games, it could be entirely plausible for him to survive the events of ME3.

I also don't think that it's "entirely plausible for him to survive the events of ME3"--in ME1 and ME2 finales Shepard wasn't alone, so there were witnesses that experienced the same (or similar) series of events that he did. In ME3, Shepard makes the decision alone, so why would any crew member believe that there's a chance that he could be alive (especially given the explosions that they were trying to run away from in the Normandy)? And like Kesak points out, why wouldn't the crew do this in Control, for example?

Why weren't there multiple beams on Earth rather than one?

like has been said..the vast majority of those are just stupid nitpicks and certainly not "must answer" questions...

why is/are "x" immune to bullets -> see every game out there..but for ME..same reason why in ME1, 2, and 3 most NPC's were immune to them. and it's the same for most games out there..if they're not meant to be killed..they're typically immune. Games that actually kill anything that moves are the exception, not rule...and that's just ONE example of these "questions" that don't need answered..because there is no answer...

why does the pistol in his hand change? same reason it changes throughout the trilogy to weapons you (sometimes) don't even have on you. y'know..like that magic rifle appearing in your hands when you didn't even bring one with you? another example.

I can go on but that's enough...a good 70-80% of those "questions" are not actually questions. inconsistencies maybe...products of how cutscenes are done..etc..but really..most of those are baffling they were even considered to be put on that "list" much less to have actually MADE the list.

it's no wonder BioWare doesn't bother answering them when that's the kind of questions they get.

-What happened to Urz?
-If Shep just replaces the Catalyst in Control (meaning Shep controls what Starchild did) then does Shep control the Reaper-Controlled Geth?
-Are animals (like Varren or Pyjak) affected by synthesis?
-Same for species like Threshar Maws?
-(ME2 related) If Kasumi is a master thief why cant she successfully open doors on the SM?
-Same for Zaeed but with leading the 2nd team?
-Why are a squad of Spectres (the best of the best) worth so little EMS?
-What happened to the Raloi in refuse? were they the Asari of the next cycle?
-How come 3100+ EMS says "chances of success are even" when they clearly arent?
-Why in low EMS (not all choices available) are the pathways gone when the paths are part of the Citadel?
-Who is Tali's mother and why is she absent from her trial in ME2?

Cinematics take up a lot of disc space, more space than anything else. The EC was mostly cinematics.

I dunno, if you sat back and nitpickingly asked questions about every game, it'd be a wonder how you'd get any enjoyment out of any game at all.

That being said, there are some major questions that need answered, but most are pretty minor questions.

I dont think I'll answer all of them, feel free to dispute them.

TL;DR Version


Since we know you wont answer all the questions we came up with we have picked out a few important ones.
 
1.Where does the breath scene take place?
---Uncertain, given is rubble and after the game ends it leads to the idea is after the end of course. Given its surroundings however two possibilities, shepard, never left london given the concrete, or he is in the citadel... the concret is awkard then.

2.Why did the Catalyst need Sovereign to open the relay?
---Some consider it a plothole, but the catalyst is not connected to the citadel, it needs outside input to connect it to it. Why? I am not sure it was never explained. Regardless this implies he doesn't not have physical connecttion to the rest of the citadel's systems.

3.How does Shepard survive the crucible blast in Destroy ending?
---Because your EMS rating is big enough. Aside from that is a pretty big plothole. Whether is because you believe the Star brat is lying or because something else we dont really know. If EMS is high enough well there is your answer. if Star brat is not lying... then he is lying. Shepard has had major cybernetics implanted into him, face, spine, most of his bone structure.

So long story short, it makes sense if you dont heed the ending writting.

4.How does Shepard's squad know that Anderson is dead?
---It is never mentioned. Only speculation can arise from this. Given the scene is shown before they leave the random planet, it is hard to know how they have had any contact with the citadel. So this creates another plothole.

5. Where is this planet that the Normandy lands on in the end.
--- Your guess is as good as mine

6.Why does Anderson's death effect Destroy ending?
---It does?

7.Why is it that all of our squad mates return to the Normandy?
---Just because the EC wanted to fix the big plothole as to how your squad got back into the normandy. Although in this case the double size SR-2 which is explained in lore that its easier for a shuttle to enter planets is a bit weird. Not to mention with Harbinger less than half a mile behind the Sr-2.

--Side note: Only one of your teamates was severely injured, so why does the other one return, I dont know

-Why when Shepard was blown off his feet in the explosion in the committee chambers, did Anderson escape - despite the fact he was stood right next to Shepard?

---He isn't on a secure military building. After the innitial explosion anderson and shep make their way across the rooftops to another building. We dont know how the 3 buildings are connected. Comitte, Child and the 3rd where shep sees him in the vents.

-How does the Child then pass through that locked door again moments later?
---He is in the vents. A million ways we could come up with how he did it, or if he infact is there.

-How does the Child survive the blast from the Reaper's weapon?
--What blast? If he does exist he was blown out of the sky on the shuttle.

-Why do levels in Core engineering building are numered from top to bottom?

Citadel (not necessarily the dlc)


-Just how is the citadel meant to function as a mass relay to dark space?
---It is never implied. We know only of primary and secondary relays that require a two way connection. It may be that there is a relay outside the galaxy, or maybe the citadel is a very unique relay that can open a wormhole to another specific point.
Based on lore however we only know relays need another relay to link both points in space

-why are there 'oily shadows' in displays all over the citadel?
Unsure which shadows you mean, I only saw them ind reams.

-How do you move an object the size of the citadel so far in such a short space of time?
--It is a relay for one, and second you have the relay network. You can move from one side of the galaxy to the other in seconds. Size is irrelevant for the relays. Exception however is density OR a prior call to pass the object through.

This is alluding to Alpha relay. We know the relays are locked on a quantum level so they are blind to the outside in every way except signals. This might mean they dont have any radars or ladars and thus require radio signals to let them know a ship is coming.

If they dont need a signal then it leads to a second theory in which they have a density limit. This is counter to the lore we already know, given asteroids were used in krogan wars, tolled to terra Nova etc.

Given the lore that they can transfer asteroids, it means they simply need to be let known that an asteroid is in need of passage.

-Where is the decision chamber situated? any shot of the underside of the Citadel has shown nothing under the Citadel tower.
---decision, as in the Council?, at the top of the Citadel Tower. We know this from ME1 how Soverign's tentacles crashed into the top portion of the tower and it went right into the council chambers


-What happened to everyone else on the Citadel during the reaper's pulling it to earth? did they survive? Gamble (or someone) tweeted some of them will have survived, how?
---We do not know, many things can be speculated
Two things we know. C-sec HQ controls the wards of the citadel itself, but only the Citadel Tower has access to everything, including the relay network. Given The citadel was closed, but the relays are still working can lead to many conclusions.


-How did they get the Citadel to Earth? How does the Citadel move?
---we do not see the citadel under propulsion, only speculation with this one. It may have engines of its own we do not know.

-If the Catalyst is controlling the Citadel (or if the Citadel is his "home"), why did he need Sovereign to open the relay for the reapers?
--It doesn't, it told you Star Brat is separated from the rest of the citadel. How it is never explained.

-Why does the Citadel have an area that so closely resembles the engines from the Shadowbroker's ship?
---Unsure, the discharge panels are similar though I agree.

-Anderson comments that the walls of the Citadel are moving and rearranging themselves. Why don't we actually see this happening?
---Why indeed

-If the Catalyst was always in or on the Citadel or Crucible and truly is an Reaper ally or designed them. Why didn't the Reapers know that the Citadel was the key? Why did they need the Cerberus tip?
-----The reapers new of the cucible designs, but they didn't know how it needed the citadel to work. They never did, Prothean VI tells you this.


-Why are there billboard posters of Kai Leng, the Shadowbroker and Jack visible during the Citadel dlc?
----No, clue

-What is the objective of the Cerberus coup attempt on the Citadel? It takes place well before we learn about the Catalyst from the Prothean VI.
---Unsure. All we know is that councilor Udina would have the council grant him emergency power under threat of death and then he was thinking to unify the races through force to send a fleet to earth. Cerberus never states if that was indeed their plan or if they had something else in mind.

-What does Udina hope to gain by helping Cerberus, when he already has Shepard doing everything possible to secure allies for humanity? Aren't Udina's actions totally counter-productive to Shepard's efforts?
----Udina is a politician, and one plan isn't enough for a politician. IN the end we dont know.



Normandy


-Why does the Normandy SR2 have alien languages all over it, when it is an earth vessel constructed by cerberus?
---What alien language?

-If the SR2 is moving ‘faster than light’ then how come Shepard can see stars out of his window, when he would not be able to see anything?

-What are the voices in the background that we hear after the Rannoch missions in the War room.

Priority Earth


- If we can blow up reapers using Cains (see also ME2) then why not just fit cains to the fleet ships?
---We can't blow reapers. We only blew a hades cannon. it implies they are vulnerable as they concentrate on being exclusively AAA batteries.


-How does Mr Random Soldier dude know everyone you want to contact by name, and their exact whereabouts and how is he able to actually contact them right on the spot?
---He doesn't know , you tell him who you want to contact.


-Why did the Reapers show such interest in London? London is a relatively small city, on a small island. If they were always planning to bring the Citadel to Earth to grow the new Reaper, why didn't they choose a city with a bigger population?
-----Speculation can arrive from this, far more than usual specially if you take into account tactical planing as Britain is an Island.


-London is also at a northern latitude. For the Citadel to remain in view of London, it must be in a geo-stationary orbit above the equator. Why then, does the beam point vertically upwards, instead of pointing at an angle due south?*
----Nobody ever said they could see the Citadel with the naked eye. Geo Stationary orbit applies to our understanding of physics. This is sci-fi. The beam may act as an anchor as well.

- How was the Normandy able to respond to Shepard's call for help so quickly?

----We dont know what the horn sound is. It may be communication it may be something moresinister.


-What caused the transport to crash before the beam run?
Multiple vessels headed for the beam, capital ships and destroyers. Any number of them could have shot them down. Even harbinger.


-Why run head first at your enemy providing them with easy shooting practice?
---There was no other way in from what it is described.


-What exactly is this beam of light? Is it a one-way transport? Are there any other beams like this in the MEU? -Why does the beam remain post-ending (e.g., it is depicted in High-EMS Destroy stills in the EC)? How was this beam not destroyed in the blast?
It is an anchor and an elevator. The beam isn't physical is a sci-fi transporation beam. It gets shut off or on it doesn't get blown up.

-Why are there so many dead bodies around, all wearing either Kaidan's or Ashley's ME1 armour?

-Why are the Reapers bothering to beam dead bodies up to the Citadel, when they need live tissue to make a Reaper?
They dont need live tissue, they need tissue. Live tissue is preffered however.


-Why does Shepard meet no resistance from the Reapers while on the Citadel?
----You only arrive at morgues, meaning the beam was not meant to transort anything but dead bodies.


-Why is there is single dead cerberus trooper in the citadel at the end? If there was cerberus at the citadel - where are the rest?

If the reapers really just didn't know, then how did they not know?
------------Wrex and others have alluded to the idea that the idea of the crucible was not being hidden, it was its location. Constant rotation and utmost secrecy. The reapers number 1 tool is surprise, in this cucle they didn't have it.

Why did none of the brilliant scientists working on the Crucible notice this? It seems like it would be obvious to anyone who was familiar with the structure of the Crucible.

If all they are interested in is preserving life, then why do they allow us to attain a level of technology where our weapons begin to pose a threat to them, and the civilisations supposedly preserved within them?

------ They need life to evolve to a certain point, and in enough numbers. The original blueprint was based on the dark energy theory. On top of which they tried to star the cycle two thousand years ago with the rachni and failed.

Also the weapons might pose a threat but their plan is not to fight evenly, rather take civilization by surprise and isolate them within their systems. Without Access to Element Zero most ships and weaponry is render useless after long periods of times.

My PERSONAL opinion (note: NOT FACT) is that about half or so of your questions are answered in-game, about another quarter are unnecessary for the story but are definitely points of curiosity, but the last quarter definitely has some things that should be answered.

For example, stuff about Sovereign/Catalyst plotholes, Leviathans fearing Reapers, the ending plotholes, and whatnot, have been answered before. In a nutshell, the Protheans screwed the signal from the Catalyst to the Keepers to open the Citadel (Sovvy sent a signal to the Catalyst), the Leviathans cannot compete against tens of thousands of Reapers at once, and so forth. But there is definitely some valid stuff in there.
