Oh there are quite a few throughout the trilogy but I guess that's not really avoidable in a game where the protagonist is voiced. The only one that really sticks out for me is the catalyst conversation, especially "So the Illusive Man was right after all."
That one just physically hurts whenever I hear it.

Do you mean with the VS in ME2?  Yeah, that was a very frustrating moment with the limited dialogue available, especially since they don't talk again until they randomly bump into each other in Vancouver.

Mordin: I made a MISTAKE!

Me: Really? What mistake was that, Mordin? Did you get the numbers wrong somehow? Do krogans suddenly not have a default, pre-genophage birthrate that's around 10,000 times that of humans? Does the genophage not currently allow an average of one viable birth per year per krogan females, which with their lifespans still allows each one to have about 10 times as many children as a human female, each one of which can live about 10 times as long? Were the krogans actually not dropping asteroids on planets and beginning to overwhelm everyone the last time around? Do you have any good reason to believe krogans will just arbitrarily control their own birthrates forever now, despite the fact no other species has ever done this? Are you really relying on Wrex and Eve to somehow do all of this for you when their entire powerbase is built around control of access to fertile females and will evaporate the instant you cure the genophage?

What ... exactly ... was your mistake, Mordin?

Actually, the genophage is now making the krogan go extinct. Which may be a retcon, but whatever.

Even Wrex admits it's the krogan themselves doing that, if it's actually happening at all.

Frankly, though, I'd choose krogan extinction over curing the genophage.  Tough call, painful call ... but better krogan extinction than everyone else extinction.

He says that in ME1; the narrative has shifted substantially come ME3.

I just been there again yesterday. I have the same though ever since the very first time I played it:

Shepard: "Udina has deceived you all. He collaborated with Cerberus in an attempt to assassinate Councilor Valern!"
Udina: "You've got nothing to prove that!!"

*absolute silence*

Shepard: "...yeah, that's what an innocent man, who's absolutely caught off-guard by my accusations, would say."
Udina: "Uhm, I meant... uh.. What? No, Valern is my FAVOURITE councilor! And.. and... the Commander once said he sounds Armin Shimerman when he talks!"
Shepard: "Claim dismissed."

ME3 has a wide range of horrendous dialogue. Despite tough competion for horrendous moments i have to say into to ME3 takes the biscuit

Shepard never calls Williams on her bad behavior on Horizon. He never calls her on how she treated him like her grandfather was treated after Shan Xi given that both faced choices beyond their control. He never addresses that she abandoned him while he was incarcerated because she was ashamed of him. He always lets her slip by with a wink and a nod and that just chafes. Williams' nickname should be slip 'n slide because she just slips and slides out of responsibility for her actions. That is the one moment I wish that I had been the writer.
