I've completed Aveline's act 1 quest "The Way it should be" but I haven't gotten her Guard-Captain armor. She still has only her Guardsman Plate and Chain. Sorry if this is a stupid question, but should it have appeared right after she was promoted or will it appear in act 2?

It won't appear until Act 2. Technically speaking, that's because she hasn't been officially promoted to captain as of the end of her Act 1 personal quest, as Bran mentions there's a few formalities to take care of.
