That's what has been floating around. I haven't seen any confirmation, but you can see Jack through her.

Also, shouldn't the title say, Jack's face based off model - Candice Neil?

Kaidan's face model remained unknown for like 5 years. It was just luck that someone who knew him had a relative who was a Kaidan fan and that's how the dots connected.

Also, isn't Joker's face model also unknown?

I always assumed Joker's face was just modeled after Seth Green. It's not perfect but pretty similar.
