I hate Phantoms much more than any other enemy, so the most pleasure I get from this game is destroying Phantoms quickly and easily by:

1) Acolyte w/ Incendiary IV > biotic charge (using my favorite, the Batarian Brawler) = instantly dead Phantom
2) Cloaked Asari Huntress > dark channnel = a Phantom who is slowly tortured to death (the fun part is listening to her groan in agony, while I hide safely in cover)

I play PC and it happens every now and then.

Yeah, i was told it happens from swarmers and acid puddles. I never knew that, and it is silly that that's what they call it.

Have dubious hour of being killed numerous times by banshee's DoT, bees, spiders, acid pools, but it never came up as "committed suicide" since. IIRC at that time I was reviving right into the crowds of very angry enemies surrounding my corpse and trying to missile my feet to get out of that mess. Even had me thinking for a while that Cobra had FF splash damage and that's what took me out... wrong obviously
