For me it's a toss-up between Kaidan or Ashley. Although I think the whiny douche-fest that is Kaidan in ME3 nails his own coffin in that respect. No matter what character I play, male or female, renegade or paragon, Ashley annoys the crap out of me with her xenophobic and religious BS. And quoting Tenneson and Whitman without even having read them herself? What a pretentious ho.
And then there's Kaidan. OMG, there's just no way on earth I believe any of the options I have to choose in order to get him on board. And him as a Spectre? Are you kidding me? He gets crippling angst over whether or not he should eat a Mars bar or a Snickers fer cryin' out loud!
So, which NPC do you find the most annoying?

With his whiny cry-baby voice "Oh, you are with Cerberus now" wish I could shoot him right then and there.
Don' worry, he'll get the bullet in ME3, looking forward to it. That voice!

Hmm... Probably Jacob with his smug holier-than-thou attitude. He was rude and standoffish enough in ME2. I wish there was some kind of response to his rude "The Normandy is your real love" line when talking about having a life. He does become a little likeable in Citadel, but his not-so-endearing awkwardness doesn't improve.

Shepard can be bloody annoying at times.
He's always falling off stuff, walkways collapse as soon as he steps on them almost sending him plunging to his doom, masonry crumbles under his feet, structures collapse as soon as he enters, sending him sliding into a pit.... Seriously.. how many times we see him falling down on his ass in Mass Effect 3? It happens with depressing regularity. The guy is jinxed I tell you!
He couldn't even organise a simple convoy along a straight road on Tuchanka, without an errant Turian fighter being shot down, and - in a stroke of insane and ironic bad luck - happening to crash precisely upon the convoy, wiping out most of the vehicles and their occupants and effectively scuppering the entire plan to deal with the Reaper. Talk about being a Jonah!
As if that wasn't enough, upon reaching The Shroud, a huge, sleek, high-tech spire that appears to be in pristine condition, the whole edifice mysteriously begins to crumble fall apart for no obvious reason.
Even when given a clear warning, such as when painting the orbital bombardment marker upon a pair of blast doors - when advised to get clear of the blast, our hero chooses to ignore it and stand teetering upon the edge... Oops! Down he tumbles onto his back for the 500th time!
The real miracle is that, when ascending the steps to accept his Spectre status back in Mass
Effect 1. the whole lot doesn't give way beneath his feet, sending him tumbling back down into The Presidium!
Can we have a none-clumsy, none-jinxed hero in the next game please?
