In more formal situations or with strangers, or wherever appropriate, I hope the title is used.

Most likely. Many futurists believe that it's inevitable anyway. I happen to agree with them.
Not that there wouldn't be a need for real humans, though. I can imagine a future something like this: an up-and-coming young actress, think a future Scarlett Johansson or something, can take up "voice modelling" - where she licenses off her vocal likeness to any licensee that cares to pay. The licensees can then slap her name on their products, which due to her star power would sell more.
Mmm... ...a Scarlett Johansson-voiced Asari LI for Mass Effect 6... Sorry, what were we talking about?

Why should these questions be considered?
Well, at the very least it may determine if the hero is called by either 'title' or last name. It also may be both depending on whom the main character interacts with. A personal acquaintance? A higher likelihood of a personal name, discussion, or what have being address. Different relations could evoke multiple scenarios when being involved in ncp's issues or simply day to day life. Player's actions as an Inquisitor may alter the mindset of npcs if distinguishable interactivity is apply, such as subjection to violence, pleasantries, and gratitude.

It should be both first and last name pretty much exclusively.
Honestly, and sorry but I had a long day so bear with. I effing hope god they just remove the first name change option. It's so damn stupid, really. Does a character whos only really unique option is first name change feel more like yours just because of it? If I hear one more person say champion this, or hero of that I'm gonna freak out. It's really annoying, especially when they refer to them by those ridiculous titles! Just saying I personally hated it, and that I need some coffee.
Dragon Age 2, Deagan says something along the lines of the Warden Commander ( I REALLY hate this one, I'm sure you guys do to ) returning shortly. Allistair replies, " He has a name you know. " EXACTLY . Tired rant over.

I like this idea, preset the names so we can hear other people in game calling the pc by the first name instead of hey you, but how many names would you need, don't forget the voice acting budget costfiletemo wrote...

However, I would find it more than a little disrespectful of random NPC#45687 calling me by my first name. The whole first name thing for me is a way to differentiate saves, and that's all it's really good for. My first wife called me Hon more than anything else, with some colorful expletives mixed in, hence "first wife". My second wife had even more colorful names for me. My last GF used a pet name, not going to repeat it here, and sometimes by Mr. Surname. So it doesn't bother me in the least bit that I'm going to be addressed by a given title, or surname. I just expect that's going to be the case.
