ME1: I used mostly Garrus, Liara & Wrex. Tali and Ashley got limited time, and I never really used Kaiden ever.

ME2: I mostly used Garrus, Tali, Kasumi, Miranda & Grunt. Samara got limited use, everyone else didn't get use outside of the loyalty missions where they were mandatory.

ME3: I used Liara and Garrus just like ME1. Ashley and EDI got limited use, Vega and Tali only got use when they were mandatory. Got Tali way too late in the game. By the time Rannoch was over the home stretch was starting and I needed my go to team, Liara & Garrus.

Seriously, their powers blow. Jacob is also boring as paint drying.

ME3 - Javik, Ashley
I liked to alternate between everybody else, except them. (I definitely used Kaidan more.)

Since some have done so here is my most used squadmates:

ME1- Liara/Tali and Wrex.  I love having a krogan meatshield and picked between the other 2 whomever was going to be my LI.

ME2- Garrus/Tali and Grunt. Again Grunt now as krogan meatshield.  Garrus was fun and better written as returning character, and awesome combat quotes.  Tali my LI with combat drone acting almost as a 4th squadmate.

ME3- Garrus and Tali again according to the Citadel DLC.  Took Tali & Wrex Garrus whines, took EDI & Wrex Tali whines.

 In #1, I really found them all interchangable. I just needed someone with a high enough Electronics and Decryption to open locks. That's it.

In #2, I rarely used Jacob, because his talents just weren't that useful. Unless I knew I was going against mechs, I also rarely used Tali, at least, not until her loyalty mission was complete. 

In #3, I never use Liara. She's forced on me for three missions as it is, and that's three missions too many.

In 1: Wrex, Ashley

Not because he's not a total badarse, but he really doesn't mesh with my characters' personalities for the most part. And of course: Ashley. I just won't have that following me around. She disintegrates anyhow. This goes for all of my Shepards. Wrex stuck around in-team a bit more though with my Spacer War Hero MaleShep.

In 2: Jack, Miranda, Thane, Samara, Grunt, Zaeed (In order from most partnered, to least)

Jack.. could probably do with some clothes. I wanted to give her a hug and send her to a therapist more so than bringing her in to battle.
Miranda.. I love her and her utter hotness, but personality-wise: I couldn't take the gung ho Cerberus worshipping at the time. Felt like I was being spied on or actually affiliating with Cerberus of my own choosing.
Thane.. He's a nice guy, but I just never had any real interest in him either way. Reminded me way too much of some amateur poetry readings I've been dragged to.
Samara.. She's nice, but again, not as interesting personally.
Grunt.. Pretty awesome, really hilarious.. Just not really into brute force so much.
Zaeed.. very, very much not in my personality to associate with. He's cool and all.. but just.. no.

In 3: Javik, Vega

Javik was just so much of a troll. I couldn't bring him along! Haha. I'm troll enough for the both of us anyhow. Made it really hard to stay in-character when he started to draw that part out of me. Vega was a nice bro and all, and I loved his open-minded personality, but I just never really brought him along all that often. I'm more about the talents than the brute force I guess.

The least used characters? Everyone save Tali and Garrus. Once I have team dextro everyone else gets cabin fever as they never leave the ship save when I'm in port.

^ I agree. Kaidan was always my LI in ME1, but he was pretty average in combat. Basically, he was just a glorified bench-(bed)-warmer. I even let him take the proverbial bullet on Virmire in some playthroughs, so my Shepard was free and clear to do whatever the bleep she wanted.

I never took Tali, either, in any game, unless I had to. I tried a few times early on, but I used more medigel than I'd care to recall, trying to keep her alive. She and Kaidan got pretty close before his demise.

To sum it up:
ME1: Tali and Kaidan
ME2: Tali, Jacob, Kasumi, and Thane
ME3: Tali, James and Ashley (or Kaidan, if he was still alive)

On another note, I was really surprised how much I ended up liking Javik and Grunt.

In ME1, I tried to keep my squad balanced, or prioritised for the task at hand. So I got a decent amount of use from everybody.

I never used Jack & Jacob in ME2 unless I was forced to - I already have a better Vanguard in my squad at all times, having another doesn't really help, especially since they're both weaker than Shepard. Everyone else, I really tried to use at some point. I should note, I made sure everyone had a role in the Suicide Mission, constantly changing my squad so everyone got some use.

In ME3, I didn't use Tali after the Rannoch missions because her speciality is the Geth - I have other squadmates better suited to Cerberus & the Reapers; I didn't use EDI that much either, but mostly because I never needed someone with her skills. Once Javik & Kaidan are available, I never touch Liara, either - Dies too often for my tastes.

Mass Effect One: Kaiden. He just wasn't all that interesting to me. Not when I could bring Wrex or Liara!

Mass Effect Two: Jacob, as I found him rather dull. Legion because he arrives far too late. Tali because I could never pick her over Kasumi, even though Tali is awesome. Thane, and I'm not really sure why. Zaeed, and again, not really sure. Mordin because he was squishier than a bowl of jelly.

Mass Effect Three: Vega. His nose really annoyed me. It's a stupid reason, I know, but it was just so pointy.

Mass Effect 1:

Ashley - There is very little that I like about her, and as such felt no desire to bring her along for any missions.
Wrex - I really like Wrex as a character, but not enough to actually use him.

Most used/preferred - Liara, Garrus, Kaidan

Mass Effect 2:

Jacob - A rather bland character.  I don't harbor any negative feelings for him like I do with Ashley, but he doesn't excite me either.
Jack - She just scares and creeps me out.  So much so that I don't enjoy her being in my squad.  I do feel sorry for why and how she turned out the way she did though.
Grunt - Not a fan of bloodthirsty krogan.  Which unfortunately encompasses most of them save for maybe Wrex and Eve.  I don't really find him that much fun in combat either.
Thane - The guy pretty boring to me, even moreso than Jacob.  I am probably speaking blasphemy.
Mordin - I really like Mordin, but from an RP perspective I think he would prefer to stay in the lab and continue his research projects.

Most used/preferred - Samara, Legion (so damn cool), Miranda, Garrus, Tali

Mass Effect 3:

EDI - I really like EDI but not the sexbot appearance.  Never found her particularly fun to bring into combat either.
Vega - Cool guy, but when you have badasses like Javik and Garrus to choose from... well.  It's an uphill for him.  Not to mention there is also the incredibly awesome Liara, Kaidan, and Tali to choose from.

Most used/preferred - Liara, Javik, Kaidan
