To begin this, I usually play on Hardcore (Insanity is just too insane for me. I die too much and get frustrated and angry) mode for the challenge of getting through the combat.
But there are times when I just don't want to be bothered. Sometimes, I'm just tired. Sometimes I'm just trying to get through the game so that I can hear a certain portion of dialogue. Sometimes the quest/combat/boss fight is one that I don't like playing (like the restaurant encounter in the latest DLC).
At these times, Narrative is my best friend. I can drop into "Gosh Mode" and get through some part of the game that I want to skim through to get to a "good part."
And then there are times when I just want to go into "Rambo mode" and punch the heck out of everyone with my Omni-tool in the certain knowledge that it would take an atomic bomb to kill me while in this mode. There is a certain satisfaction in living the legend.
So thank you, Bioware. It's a great idea, I loved it, and I hope it shows up in other games.

Honestly, in my first (and only full) play through, I dropped it down to narrative mode for the final Banshee fight, lol.
I'm now working on a second play through after a year and a half if multiplayer, and I can't believe how easy the whole thing is on Hardcore. We'll see about those Banshees at the end, though

Multiplayer experience and DLC weapons help a lot in that fight, if you give the Typhoon to your squad Banshees die almost too fast to shoot them yourself.
Once you are aware you only have to not die for some scripted event to happen until you can fire that Thanix rockets, it's easy to do.
If you want a real tough fight, get Citadel DLC

^On Insanity with the right squadmates (there are a few) and the right weapons, you can basically just sit behind the trucks during the defend missiles sequence and wait until EDI to tell you to push the buttons. Your squaddies will chew everything up for you up until the last part with the mass Banshees; but only b/c you'll need to run from the Reaper beams and can't sit around waiting for stuff to die.
