... I don't switch to Insanity until AFTER Lazarus Station?
I escaped Lazarus and went to where i met TIM. Before I did, I set the difficulty to Insanity.
Will the next 20+hours matter if I'm going for the achievement?

No, I think you have to set it to Insanity before you begin the game to get the achievement.

Can anyone verify that for us?
Just because I think both ME1 & ME3 you can finish Eden Prime and Earth, respectively, on the normal mode and then switch.

You're correct about ME3. Just have to set it to insanity before getting on the Normandy.
ME1 and ME2 have to be set at the insanity difficulty before starting the game.
