I think that the game will begin with a battle (like Ostagar in Origins). The battle will result in a tear in the veil as seen in the trailer (possibly with some help from "Agents of Chaos"). Combatants will consist of factions involved in confict between mages and templars. Outburst of demons will force everyone on the battlefield to either fight, flee or die to outburst of demons, possibly they will unite againts the forces of the fade.
During the battle the protagonist will meet, perhaps even team up with, companions that are important to the plot representatives of factions present during the battle. I think that those companions will be: Cassandra, Cullen representative of the mages and representatives of other present factions. I think that the Grey Wardens might be present, whatever the form of their involvment (they may be present because of dark spawn activity in the area or they they might be involved in the conflict through Fiora and perhaps even Anders).
The rest of the game will be the ussual BioWare formula:
Gather allies: solve their problems -> get their assistance in the final battle.
Something between the gathering of allies and the final battle.
Final battle.
End of the game - the ending will likely make all the choices from Origins and DAII unimportant so the choices from Inquisition can be carried forward on to the next game.
I suspect that Flemeth will be behind dragon activity. Dragon activity might be connected to the "Agents of Chaos" who might be dragon worshippers.
Oh... and Morrigan saves the protagonist at the start.

The protagonist origin will be roooted in the of the factions present during the battle.
