no insanity trophy...WTH!?!

So after finally getting bored of multiplayer, I had finally downloaded all the SP DLC I hadn't previously played and set out to finally do an insanity playthrough.  I hadn't played the SP campaign since last summer.  As a PS3 player, I started with ME1 for the first time and went all the way through the series.  Since I'm a completionist and also have a family and a FT+ job, it took awhile.

Anyway...I finally finished tonight.  The only trophy I had left after completing the DLC trophies was Insanity.  Frankly, it was a breeze.  After Gold and Platinum MP, this was no challenge at all.  I never changed the setting, and still the trophy never popped after completion.  Can anyone tell me what might have been the issue?  Before realizing this, I deleted some of my stored saves, but I still have my last Priority: Earth save if re-finishing it might help.

Here are some possible causes that popped in my head:
1. I took the refuse ending.  I told that starchild ****hole that I wouldn't accept his stupid choices and then got to watch the Liara time capsule ending.  It was my first time to take that route, and I wanted to see what it was like.  Could that be the issue?
2. I didn't wait through the credits.  Ain't nobody got time for that.  I hit whichever button it is to bypass it and it brought me back to the point right before Cerberus HQ, as usual.  Then I went to the main menu.  Should I have waited through the credits?

I reloaded my last save to double-check the settings.  Confirmed it is still on Insanity and I have never accessed that menu during the playthrough other than to adjust the stupid helmet on/off settings.

Other ideas?  What gives?

I think your problem was in choosing the refuse ending. I remember seeing a similar post about this exact issue before, and the person had chosen the refuse ending. Once he went back near the end (I think there is an auto-save right you can use) and choose one of the colored endings, you should get the Insanity trophy.

I think the issue is that the colored endings are the original endings and have the ability to trigger the trophy/achievement (there is code for that), while the refuse ending was added by the Extended Cut DLC and has no such coding to trigger the trophy/achievement.

I have to say, by not giving you a completion trophy for Refuse, they took the trolling for that ending to a whole new level.

Haha...true.

Ran through the end again with Destroy and got my final trophy.  Thanks again, Red.
