Won't your very first companion will be someone who dies at some point in the tutorial mission? I'm pretty sure that's tradition.
ME1: Kaidan is the 1st introduced, Jenkins the 2nd.
DAO Human Noble: Dog is the 1st introduced, Mama Cousland the 2nd.
ME2: Jacob is the 1st introduced, Wilson the 2nd.
DA2: You siblings, hard to say who's first, but one of them dies.

No, absolutely not. Cassandra is a outside party character who has been trumpeted as some kind of medieval Lara Croft. I sure as hell don't want another Tallis.
As for Varric, he had his story with Hawke and it is done. Unless we get to play Hawke again.
