The first companion for DA:O was announced about a year previous to the game shipping because it was delayed 6 months.

I want to say that the image of Morrigan and blue ogre or the first teaser of Alistar was at least 3 years before though. They were assumed to be companions (or enemies for Morrigan) but major companions tend to be shown before the official announcement that they're part of the party.

I just remember that early on when they were teasing Origins they made a big deal about Duncan and King Callen, which is kind of funny in retrospect.
