I think the one you don't choose won't kill you but I have a feeling they'll leave the normandy and you can't choose them to be a squad mate anymore. I think this is a good penalty.

in ME3 you should have to choose between your romances from ME1 and 2 to see who lives unless you choose say Tali for both 1 and 2 that'd be cool
