Saw this thread on Sherdog, thought it would fit here too.

Star Wars: The Old Republic. Instead of wasting tons of money on giving Larry the merchant a cut scene and 1000 lines about his life and then end up having you kill 15 worms, they could've focused on making the game better.
Dragon Age 2. Freaking copy paste zones and that idiot Anders as the sole healer, anime like combat with waves.
Diablo 3. Just couldn't hold my interest, and I really tried to love it. **** you always online too.
Aliens: Colonial Marines. **** you gearbox
Borderlands 2. Never going to buy another gearbox game again, it's almost the EXACT same game as Borderlands.
The Witcher. The first one. Bad mechanics and gameplay, nearly put me off buying the Witcher 2 (thank God I did buy it, turned out to be very enjoyable).
Fallout 3. Way too boring at beginning and didn't hold my interest, quit after 30 min. New Vegas was much better, but it got boring too in the end.
Command & Conquer: Tiberian Sun. One of the most overhyped RTS games back then, even scored 10/10 from many magazines. Eye opener that showed that reviews can't always be trusted.
Spec Ops: The Line. bla bla bla, pseudo intellectual bs about war and crap and a bad plot, extremely short with low if any replayability.
Rift. Got boring quickly.
Guild Wars 2. Same.
Mass Effect 3. Cause of the horrid ending and the fed ex quest and lack of dialouge.

Not even DA2 managed to clinically depress me.

Spore, for taking a fantastic idea and executing it with tedious, borderline miserable gameplay.

Anachronox, trying to hard to mix hilarity with seriousness. Likely had influence on the Mass Effect series.

Most Star Fox games after the first. 64 wasn't bad, but the rest seemed to deteriorate. Technically Star Fox Adventure was a great game, but it wasn't originally a Star Fox game.

Megaman X5. Good lord, don't touch it.

Well I'll cast aside a few of the ones you already mentioned & go with: Kingdom Hearts 3, Battle Front 3, & KotOR 3. These games have left me with more disapointment than anything else & they ain't even made yet... also whats with 3's?

Kingdom Hearts 3 trolled my heart for an entire console generation, that was a long long time. I am glad its closer to reality now but still. An entire generation, very disappointing. And whatever happened to you never know who you'll meet next, I know licensing is a horrifying pain but I hope they switch it up some.

Battle Front 3. These games were fun, they were simple. And then this one turns into wack'a mole. Oh look it's here... & cancelled. Oh look it's... Cancelled again. But wait they're... Ah never mind. Development behind this game must be an interesting story, they could probably put together a book on it.

KotOR 3, so much anticipation. The first game was good, so good... Didn't need a sequel. Then Sith Lords comes and is an awesome addicting game. Sith Lords left me so excited. I was so ready to see where the Ebon Hawk would take the Exile in the third game, but then that ship imploded in the vacuum of space. Like all dreams. The game got cancelled, & was swallowed up by TOR.

Mass Effect 3 - ending ruined the franchise, streamlining and decisions don't matter

Dragon Age 2 - annoying characters, reused levels, RPG mechanics streamlined to death

Diablo 3 - www.youtube.com/watch and eu.battle.net/d3/en/forum/topic/4941486367

Assassin's Creed 3 - weak plot, Connor is shallow and boring, worse ending then ME3

SimCity (2013) -  EA at finest with Origins DRM and so many stuff were taken out from the game

Saints Row 3 - wanabe GTA title with lame toilet humor and purple dildos, not even close to being cool as SR2

Max Payne 3 - dark and scary New York and thriller elements got replaced by shooting in Pavela

All three are from Bioware. And all three are Back to back to back bioware games. No wonder I won't buy anymore of their products. So without any further ado here's mine

DA2- Yeah, we all know what happened here. Yuck.

SWOTOR- the tv commercials and trailers make it look f*cking amazing. But it's pretty lame.

ME3- This is the biggest. BIGGEST. Disappointing game ever for me. It's so soul crushingly weak. I've never. Never in my short life seen a series that was amazing for over 100 hours and a game that was great for 99.5 percent of it fall apart at the end. It didn't stumble to the finish line; it crashed, rolled 5 times, hit a baby seal- and puppy, fell off a cliff into the ocean, sank, got eaten by a blue whale then pooped out onto a deserted island. Terrible ending. WTF Bioware.

Mass Effect 3. I was so let down by this game, before the game came out I was really gutted. The story leaks and gameplay videos stink.

The animation is terrible. Anderson's jog or run or what ever it was, the dialogue was terrible with barely any choice, the shooting was just a copy from other 3rd shooters, turret section against the reaper, you don't even need to fire the damn thing, Earth was just a rush job and the side missions were bollocks.

Im not going to mention the ending as unfortunately, it overshadows the real problems.

Dragon Age 2: wave after wave of enemies. Spam A to win, again choices don't matter and the environment was just so static.

These two games disappointed me the most.

I've only really had one game disappoint me and that was Mass Effect 3. Dragon Age II wasn't a great game, but I wasn't expecting much because of their development time, it felt rushed and it showed, but I still enjoyed it.

Mass Effect 3

Never before in my life was I so hyped for a video-game, only to be so much disappointed with such terrible mediocrity (and that's putting it mildly, ME3's story, plot and direction is actually worse than just "mediocre" in my opinion).

I guess the joke is on me. With ME2's direction regarding the writing and  BioWare marketing ME3 towards the average dudebro gamers, I should have seen this sh!t coming from miles away...

Mass Effect 3.

Pathetic main plot, pathetic final mission, pathetic endings, character assassinations, ignoring the lore, auto-dialogue, mediocre singleplayer combat experience and many more.

NWN is interesting, because it was a major disappointment initially - the official campaign is pretty bad really - but totally redeemed by modding.

Duke Nuk'em Forever. Hands down for me.

My running gag that 'DNF' describes my play through.

Edit:

Okay... and Mass Effect 3.

When you load up the game for the first time and port your character but it's broken in a game that is all about the character, you know that that does not bode well.  You only get one 'first impression'.

Ooh, let me think.

Silent Hill Downpour - I'd already been burned by SH: Homecoming, but at least I managed to get through Homecoming a few times. It's like they forgot what makes Silent Hill Silent Hill. For me, seeing this series continue has been like watching a loved one wither away from illness. Okay, perhaps not QUITE as bad as that, but it's up there.

Final Fantasy XIII - I had never not enjoyed a FF before. None of the characters appealed to me, I never cared what happened to them. Thinking this was a rare misstep for the series, I picked up FFXIII-2. Slightly better, but I'm not waiting with bated breath for the third installment. And after seeing that trailer of FFXV, I can say with certainty that the series is just no longer for me. Thanks for the memories, Square.

The Legend of Zelda: Skyward Sword - Part dislike of the controller, (I can't fault the sword's responsiveness, though. That thing's precise) part lack of challenge, I remember being stuck in Ocarina of Time's Water Temple for days. Skyward's "Water Temple" I finished in about an hour.

Suikoden: Tierkreis - it seemed like a hollow shell of a Suikoden. Normally your headquarters has tons of stuff to do, this just had the bare minimum. You can even recruit a gambler and you can't play Chinchirorin/Triple Toss. The voice acting was quite nasty too, constantly mispronouncing names and generally rushing through their sentences. Some people thought Suikoden IV was a low point for the series. I bet they never played this one.

Theresia: Dear Emile - Got this one after it was highly recommended by a few people. I shouldn't have. When you have to closely examine every single piece of furniture in a room before touching it in case a GIANT NEEDLE OR KNIFE will come flying out of it, things quickly grow tedious. Seriously, a bare wooden chair. You want to move it across the floor? OH NO, IT HAD KNIVES IN IT. YOU ARE DEAD. It was like it was written by children. Adding the fact that your character moves like molasses, I just couldn't bring myself to finish this one.

That's pretty much it; generally, I try to keep my expectations pretty realistic for games.
