It might not be the answer you were looking for, but I'd suggest lowering the difficulty.
It's not that. I've played through Insanity 2 or 3 times now. I'm just looking for something to add to the challenge of the game.

Actually, I managed to make it through Citadel without dying (unless you count the AAA).
The Harvesters on Thessia gave me fits, though.

Sentinel, and yes, it was the run to the Maw towers. First death was 3 Brutes taking me. Second and third deaths were being stepped on by a Reaper.
I don't even want to mention how many times I died on Rannoch. Damned targeting locator...
