So I'm going through my first playthrough. I'm doing it on Nightmare. As is my custom I played every class and spec about 7 hours in to get an idea what I want to play and how to play it.
So now I'm on to Nightmare. Mostly because I found Hard too easy. I'm gandering the item packs. My question is one of impact. How much of an impact will the item packs have on my nightmare difficulty? Are they way, way stronger than what I'd find in game thereby negating the challenge of Nightmare? Or are they mostly inline with what I'd find and only negatively impact my difficulty a bit?

IMHO,in the amount of time you each charecter I don't think you've gotten a fair judgement of hard difficulty.on hard mode during early levels and enemies the
Game doesn't seem much different,but once those dreaded enemy elite rouges( or whatever they are) start disappearing and popping up behind your mage or rogue hard becomes alot more difficult.bloodmages start to melt your party.
Then you have nightmare mode is like bringing a tank to a boomerang fight.ive had this game for a long long time I've played all modes and hard is the perfect level of difficulty for me.nightmare is just to annoying one wrong move,a failed tactic or just that disappearing rogue can destroy your whole party,however some people find this to be fun.it will make dragon age feel like a whole new experience.
Now to the topic (sorry for my rambling) I don't think any weapon is game breaking in da2 (builds can be over powered) but not really anyweapon I've found.i have dlc weapons yes it does make the start of the game easy (levels 1-8),but these parts are pretty easy anyway and I just think of them as a feeling out process for my charecter.but there is strong enough items findable in game to make these levels easy.it seems to that each act has a specific set of weapons and armor pieces that help make life a little easier.
But again I don't think any item in da2 is broken over powered but that atk speed rune combined with haste lol

Have you got the Legacy and MotA DLCs?
The companion accessories are all very nice, so much so that you'll probably never bother to replace them except Merril's if you make her a blood mage.
The armor is well ahead of the item curve in terms of bonuses and armor rating. The lv17 armor pieces in particular are better than their Champion set equivalents and are pretty much the best in the game for that item slot unless you do one of the DLC modules in Act 3. This makes your armor rating ridiculously good in Act 2 when you first get to wear them, but it will quickly drop down to normal as you level through Act 3.
The level-up weapons flitter above and below the item curve throughout the game, in terms of base damage, though their other bonuses usually make them worth considering regardless. Toward the end of the game though, these weapons continue to grow in power with each level, while the vanilla item curve just kinda stops somewhere in the early 20s. So if you have both DLC modules, which each have about 3 levels worth of experience, the level-up weapons will end up a bit better than the best vanilla weapons in the game by a small margin. Doing Legacy late in the game might yield an even better weapon though.
So... are the item packs cheating? I wouldn't say so, but they will definitely make the game easier, especially in the first two Acts. Their most dramatic impact is probably not so much the items themselves, but the gold you save from never having to buy any expensive weapons or armor.

On armor and accessories I can only agree with OniGanon. Little addition about the weapons:
The weapons only become significantly more powerful than other stuff if you level very high (all DLCs). By then, level-scaling has murdered your armor-rating, so the little help is fair.
