When Cullen flipped out and told my 1st PC (a mage) to go kill everyone and I kind of saw his point after fighting my way up to him, that was a O_o moment. Hespith and the broodmother did it again, and thenthe Anvil just made me asssume all dwarves in Thedas were psychotic. And then tney actually let me kill Isolde!
DA2 had its share, too. When Justice freaks and tries to kill the birl we came to save I thought, "oh, crap". The second time through he did and I totally didn't think that would happen. I couldn't believe it when Fenris's sister showed up and then was willingly working with Denarius. 'Bela left me twisting in the wind. And then Anders, again...

-And then... When you visit the Arishok looking for those elves and he steps suuuuuper close to Aveline.

Bann Alfstanna had said, "It shall be fought according to tradition: a test of arms in single combat until one party yields." I couldn't go along with Alistair's desire to kill Loghain anyway, I wanted him to be tried for his crimes and punished/executed the "legal way." Then Riordan suggests he be made a warden and Alistair freaks out...the next thing I know Alistair says he's leaving, turning his back on his country in its hour of greatest need while completely abandoning everything about the Wardens that he had said meant so much to him. Then Anora butts in and says he needs to be executed...shut up, you're not helping! So, I sit there thinking there must be a way around this that doesn't involve putting Alistair on the throne or killing Loghain. Finally I support his execution - not because of a possible future civil war (weak assertion on Anora's part), but rather because he is betraying his nation (in my eyes) by running away right before the battle with the archdemon. I can never support putting him on the throne, I have always thought he would be a bad king and the power would go to his head (very first playthrough when I put him on the throne and his subsequent attitude at the beginning of Awakening confirmed this to me in my mind). This part always gives me a sick feeling in my stomach when I do a playthrough, but he always ends up dead...for me, there can be no other way.

The first time we tried to take the "deserted building" way too early in the game.
When that **** betrays us and we end up in prison.

That reminds me of the time I first took on Flemeth with only two dozen potions and no healer as back up, I got absolutely destroyed and was forced to come back later on.Bionuts wrote...
I went into the Deep Roads with only 30is potions, and had 0 left by the time I had to get to the anvil.

I normaly don't stock up on health potions, if I can't win a fight without turning my characters into potionaholics I would consider it a failure. A shame really beacuse I want to use the alchemy skill more.Bionuts wrote...

DA:O - The march to the final battle, gathering all your troops and realizing this is it. And also your companions farewells at the gate.
- When the Crows try to persuade Zevran to return with them.
- There are a lot more but most have been said already.
- Sparing Loghain at the Landsmeet for the first time, hearing his side of things and then letting him kill the archdemon and be redeemed.
- Finding the king's body at Ostagar and seeing flashbacks with sad music. I cried.
DA:II - Realizing pretty much every mage you've ever tired to help eventually turned to blood magic or demons. Orsino...why?! You really begin to wonder if you made the right decision.
- That blood mage **** in The Blooming Rose who almost forces Hawke to commit suicide.
- Bartrand's betrayal and seeing what became of him afterward.
- Finding the viscount's son murdered in the chantry by Mother ****ing Petrice.
- Realizing you have to duel the Arishock in single combat.

I had a similar experience. You can imagine the feeling when I figured out that you could fast travel out of the Deep Roads at various points via the map on my 2nd or 3rd playthrough.Bionuts wrote...

-Hawke's Mother becoming a zombie. Hands/arms and legs probably belong to the other women. Which I wonder where the rest of Leandra's body was. Distrubing to think about
-Single combat with the Arishok. Especially when you play a mage.

Having to sacrifice Isolde (which happened on my first playthrough because I actually failed to save the mages at the tower).
Finding Seamus in the chantry.
The entire scene leading up the the Qunari finally attacking Kirkwall.
Anders blowing up the chantry. We broke up after that.

I thought she would have some crazy spells, area spells, etc.
I went up to her solo thinking about how cool it would be to beat her 1-on-1.
Then she turned into a dragon... had to turn that chit on casual.

- "Sound the Retreat." From Loghain in Ostagar, literally a WTF moment when I first played the game. Then watching Duncan hopelessly watch the tower just burn as just... nothing. Like hope extinguished.
- Finding out about Broodmothers in Origins.
- And the Golems of Amgarrakk, literally meat golems. Urgh.
- Seeing Leske again if you played aa Dwarf Commoner and what he had to become to keep surviving. That was actually quite heartbreaking.
- Leandra Hawke. The follow up and the result. Adult fear right there.

I found the Paragon of her kind quest to be very dark in general, that's why it's also by far my favourite main quest. But this line really gave me goose bumps and made me think "This just got real guys!"
