Without access to relatively modern standards of health and cleanliness unfettered sexuality would be remarkably dangerous. Well, either that, diseases are differently in Thedas or everyone has magic genitalia.

While you were probably joking about magic genitalia, I believe that to be partially true; if Isabella gets a disease, she can go to Anders. If this happens before she meets Anders in Kirkwall, she just finds another apostate, possibly even Anders given that they'd apparently had sex before they met in Kirkwall. If a noble gets a disease, he or she gets a legitimate mage, or else an apostate. And the Circle mages probably only get STDs during sex with new arrivals, since healers are so easy to get there.
Even accounting for the fact that not everyone can find, can afford, or even would use a mage's services, diseases are bound to be a lot less prevalent with that option open. The prostitutes at the Blooming Rose, for instance, ought to be spreading diseases like its going out of style, yet they're probably clean most of the time since the Madam can almost certainly afford to have their diseases removed. So, while the whole hygiene thing might make the act less enjoyable, diseases are somewhat less of a problem than they could be.

I'd file it under personality options. You can act like a really selfish, petty arse through most of these games, which isn't terribly heroic, but they'd be weaker (or perhaps less funny) for the absence of the option.David7204 wrote...

I do agree with this.
If a player character is going to be able to have flings with NPCs it should serve some purpose in the story, and not just be there for the sake of an achievement or wish fulfillment.
