Do you think/wish some Squad members should have died in the final battle apart from at the Beam run?

It can actually happen. If your EMS is low, your two squadmates cark it during the beam run. In the same way, it's a lot like how not having them loyal leads to automatic death at the final section of the suicide mission.

The reason they're not going to arbitrarily kill off random characters without any player input is because:

A) They're not jerks.
B) People will just find a way to exploit it so only the characters they don't like die. Expect EDI, Liara, Ashley and James to be the most common 'tragic losses'.

Actually that isn't an error.

In Refuse Liara dies regardless of whether or not she survived the beam run. The entire galaxy is wiped clean of sapient space-faring life, with perhaps the possible excepting of the Yahg.

The voice over you hear from Liara is pre-recorded. Its a message she buried on one of the time capsules, in the event that they were defeated. The Refuse voice over is basically a reference to Liara's 'Name in the Stars' scene. So considering its a pre-recorded "if you're hearing this, we're dead" message, whether or not Liara survives the beam run shouldn't have an impact on the Refuse voice over. The time capsules are spread throughout the galaxy long before the Earth counteroffensive.

I actually thought that happened, since I took James and Kaidan, then heard "Hammer squad is all wiped out!"

That one Alliance dude who struggles a bit before dying... I thought it was James and was like, dammit!!

Then the Normandy crash scene happened, and Kaidan walked out. =|
