You leave the Kokari wilds and travel to Lothering expecting to enounter Dog, but instead you don't and realize you forgot to turn in the flower and Dog is dead.
Then you realize you have no saves before the battle of Ostagar and it is therefore impossible to get dog.
Damnit.

There are save game editors you can use. Personally, though I liked the dog, I never used her/him. I just liked the dialogue between Morrigan and Alistair when I first acquired the dog as a party member.

If you have Return to Ostagar DLC I think you can get Dog at the DLC Ostagar area (if you failed to finish the quest). The houndsmaster will tell you that the dog will probably recover even without the flower, but the flower improves his chances.
