I need a advice about insane difficulty... I want try to get achievment for completeing the game in that difficulty and need advices.
1: Which class and weapon use?
I thinked about Vanguard with N7 Piranha or Claymore. Also thinked about Infiltrator with Black Widow or Engineer, but for Engineer not sure for weapons. And wich mods you recomend?
2: It's worth to try get all war assets as possible in insane difficulty? Since if i good remember, reapers are faster in each difficult. Possible to scan, get item from planet and run?
3: Which tactics you recomend against all factions in single player? Since in SP, Geth Primes drones not attacking, and don't have a bombardiers until the Armax Arena. Same for Cerberus they don't have Dragoons until Armax Arena.
Other tips are welcome also!

Insanity is a cake walk in ME3. If you want maximum easy mode, take the vanguard with incendiary burst ammo, charge and nova. You can use any shotgun you want, as not much is going to survive the charge/nova combo. I used the disciple because it is light and sounds cool (it's pretty trashcan in honesty) I would not recommend the Claymore without NG+ as it weights about as much as a cruiser until you get it to level X. Talon pistol is another worthy consideration, it is really light and can easily keep pace with any shotgun in the game. If you have the DLC for the piranha (a solid shotgun choice) then you have access to the Acolyte, which is a really good sidearm for any class and I would highly recommend it on the vanguard.
Tactics for the vanguard are pretty straight forward: charge in, nova, mop up any survivors with your shotgun and incendiary burst ammo. Rinse and repeat. Use the acolyte for banshee's and Atlas if you don't feel comfortable operating within their sinc kill range. Just make sure you bring Ashley (for disruptor ammo) and Tali (for sabotage) along for the Geth missions and you will be A-OK. Liara and Javik are solid choices for reaper missions, but not a requirement.
It's pretty hard to go wrong with a novaguard.

The difference between Insanity and other difficulties is that enemies deal more damage and have more health and defenses. Other than that, everything is exactly the same, so you can use the same tactics.
I would say that the Vanguard and Infiltrator are the two most powerful classes because of how they operate: Vanguard can exploit immunity frames by spamming Charge and Nova (requires Half Blast at rank 5). Infiltrator is just insane since you can go invisible with Tactical Cloak and increase your weapon and power damage by 90% with Tactical Cloak alone.
As RA RA XD wrote, with the Vanguard, just charge > shoot > nova, and then repeat. Vanguard can use pretty much any shotgun and do well.
Even the Claymore V is good on a Vanguard; if you are concerned about cooldowns with it, you can wear armor that gives you bonuses to recharge speed. Between the Claymore damage, weapon bonuses from Charge and Assault Mastery, and Incendiary Ammo damage, you can kill just about everything pretty quickly.
As for Infiltrator, a lot of people recommend the Black Widow. I do not, but only because I do not see the point in spending the money on it when you can do just as well with an Eviscerator I. But then again, I use shotguns on just about every class so what do I know?

1) Use whatever class you're best/most comfortable with. Likewise, use the weapons you have fun using. Insanity is not a chore or a huge grind, you don't need optimal classes and weapons to succeed. Play what you enjoy.
For Engineer weapons, I like a somewhat light load-out so I can spam my powers. Hurricane, Wraith, Paladin, Talon, or Mattock are all good choices, light but still packing a punch. I generally carry only one weapon on the Engineer since shooting is secondary to powers. For mods I usually just take armor-piercing and/or extra damage mods. The Power Amp mods for Pistols and SMGs are nice too, if you have them.
2) It easy enough to get all the war assets you want to pick up. At the worst, you might have to come back to a system after you finish another mission and the alert level is gone.

Sentinel w/ Fortification is one of the "best" classes for Insanity.

I'm late to the party, but here's what I have to say:
On insanity, the rules of the game are the same, but the penalties for messing up are higher. If things are too easy, up the tempo. If things are too difficult, slow it down. You'll figure it out. I did, and I'm by no means a greater-than-competent player.

Power combos. Learn them and plan you squad accordingly.
