i used poisons, salves and bombs all the time. traps i dont ever recall using.
i ended up using salves a lot less through each play through when i got better at giving commands to each companion but i had them for tough fights and emergencies.
poisons and bombs were very useful, and as i almost always play rogues (even in subsequent play throughs) it's more "in character" for me to use all the tools available to me to get the fight done fast and move on rather than making things more difficult for myself for the sake of a "challenge".

I used them a few times in DAO but found them not worth taking up a shortcut button and going in and out of the menus was certainly too much trouble for such little effect.

I never gave them a fair shot in 2 because of the first game. Generally speaking I doubt I'll be using them in DAI either.

I used them some, more so in Origins than in DA2. In Origins poisons especially made sense, being cheap and easily available while having a short duration. In DA2 they became a pain in the behind, since they were relatively expensive and laster for half an hour. Since I spend way more time talking and running around than fighting, it struck me as a monumental waste.

Grenades I used a little bit, but again mostly in Origins. Too expensive in DA2. Salves, only for dragon fights.

One thing I sorely missed in DA2, though, was traps. I loved setting traps in Origins and was constantly running out of shrapnel.

The last time I made a trap was the one time I did it in Lothering with the tutorial/npc lady who needed them.

Salves not really, but bombs and poisons, yes. They were actually pretty essential to my canon Rogue's style.

Yes to the poison and grenade part. My "canon" character loved poisons, so it was a roleplaying experience.

Not really. By the time I realized what those things might be used for (aside from just straight-up healing and mana potions) in DAO, I had already beaten the game more than once so never saw the point in using them.

In DA2 I usually forget they're there, just because of how the crafting/resource system works.

Also I tend to play mages, which means most things that can be done with grenades and traps and such can also potentially be done with a spell.

Salves for dragons, poisons when I got 'em, bombs I hoard and never use. (only ever remember using them significantly against the Harvester, them rainbow colored bombs from Awakening, those were neat)

Grenades were fairly useful on DA2, the revive one I used a lot against that darn elemental near the boat back to the gallows on Act 3. Also used it as a way to save mana and deal a little damage. salves were pretty useless.

That's my answer is well, though I'd add that grenades and salves were uncessary in DA:O regardless of difficulty.

I used them a couple of times just to try out their effects. I would really love if the poisons were able to be applied to a weapon permenantly(like an enchantment in skyrim) Or at the very least remain active for quite a while.

I can understand the consumables might be useful for nightmare difficulty but on hard and below it really wasn't worth the extra micromanagement.

Not sure what to do about salves... put them on your armor...? But I am sure something could be done to make them more rewarding to use.

I don't think I've ever used alchemy in any game I've ever played...except for the Witcher because it was necessary for immersion. Blacksmithing is probably the most popular in my opinion.

This pretty much for me. 

Used to craft stuff all the time when I picked up stuff in the field in Origins.  That would remind me that I had them and I would use them all the time.  Sometimes right after I crafted it.
