So I was just wrapping up my ME2 playthrough, and in one of the lines of dialog, when Shep was giving one of his moving Paragon speeches to the crew, he said something like:  "we've already lost one, but now we say NOT ONE MORE!"

...But I don't think I lost one.  When I finished the game, indeed, all the crew was aboard the ship - there were no cutscenes showing I lost the crew. When I got back to the ship, the crew all was congratulating me on rescuing the crew and defeating the Collectors in time to save them. All of the crewmembers appear alive as well. I found that suicide mission guide on the forums, and think I followed it, so everyone should be alive I think... but then why was there that line of dialog that seems to indicate someone died?

I think he says "some" not "one" and he will say this regardless of whether you lost any or not.

I think for the 'suicide mission' to have dramatic weight there must be casualties. I'm glad there is a way to do a 100% survival trip, but for 'serious' playthoughs it cheapens the storyline if everyone made it out alive. My 'canon' deaths feature Thane, Legion and Mordin not making it back.

It's like when shep and TIM are about to talk about the fate of the collectors. Shepard says: "I was part of a team. Some of them gave their life for this mission" Even, if no one has died.

Don't some random, nameless crew members die during the Collector attack? I always thought such lines were about them.

This.

The only way for anyone to actually die is for you to be a incompatent leader, which makes me question why Cerberus would bring you back to life in the first place.

Or you could be a sadistic SOB. Like Reynold Sheppard who plans on seeing Zaeed and Jack die.
