If everything could be done over, I'd have to go back all the way to the start of ME2.
Shepard doesn't get spaced, nor is there a Project Lazarus. Instead Shep gets marooned and hurt badly. The first part of the game might've been some fun survival based antics and Shep trying to patch her/himself up alone on a planet. I'd give it 6 months, surviving alone. At the end, he'd grow a long beard and start talking to a volleyball.
"Wilsssson!!". (See, Wilson is a volleyball. Not the Cerberus medical technician from ME2).
And then Legion shows up. For the same motivations he had in ME2. He ends up helping Shepard and makes him aware of the Heretic problem. And the dark energy problem, more importantly. The first part of this story would be Shep getting used to the idea of a talking Geth, and his warnings about dark energy and how it's connected to defeating the Reapers (this will also tie things closer to Tali later). After all of this, Shepard manages to get back to the Citadel with Legion's help. At this point though, the Council and Alliance had counted Shepard for dead and said it was due to a Geth attack. Now Shepard shows up with a Geth and starts talking about allying with them.
The whole Virmire Survivor situation would revolve around conflicting on this, rather than Cerberus. People suspect Shepard isn't really Shepard anymore and some rebuilt cyborg.. a ploy of the Geth. They arrest him and want to do further tests.

Actually.. I'm going to stop here. It's an entirely different premise and would take too long to write. Needless to say, I'd have Cerberus as the enemy right away in the second game. I'd have Shepard a fugitive, suspected of being a Geth clone, an enemy of humanity... Shepard would still assemble a ragtag team all the same, but for different reasons. And he wouldn't have the Normandy SR-2. It'd be a Geth ship. Eventually Joker would come on board, but there'd be no EDI. It'll be Joker and Legion instead.
