I'm really confused. I've played DA:O multiple times and have yet to receive the Master Warden or the Blight-Queller achievements when I know I've completed what was required for it. Anyone have a clue what I may be doing wrong?

It is a known bug - introduced by one of the patches no less. Removing the latest patch usually helps (can be reinstalled later).
I got these way back when no patches were around and these worked just fine.

I've been playing on the PS3 and didn't get the achievements. I haven't been playing the PC version long enough to get the achievement on that. Wasn't sure how to remove a patch on the PS3 though.

I'm not sure if all the kills have to come from the warden directly or not, but I remember that you have to get the 500 or 1000 darkspawn kills in a single playthrough. It will not unlock if say you have 600 kills on one warden and 400 on another, must all be on the same character.
Check your Heroic Accomplishments, how many Darkspawn does it say you've killed? You might have to go out of your way to do side quests for more kills.

Blight-Queller is bugged and will not unlock even if you do kill 1000 darkspawn on one character. I even soloed the game and killed well over a thousand, but it still didn't unlock.
It's annoying, but there's nothing you can do about it. As much as I love DA:O, it suffers from some really irritating bugs.
