its already been established that the PC is going to be the first Inquisitor so I assume the title will be "the" rather than "an"

Likely, we'll see the Veil Tear. And all kinds to crazy baddies start pouring out. We'll fight with a group of others, seeing first hand the power of these never-before-seen demons and dragons. Our team will either die for being inexperienced (the Jenkins treatment) or wind up sacrificing themselves to allow us to escape (the Gorion treatment).
Then, the Seekes and Cassandra will arrive on the scene. Seeing that we witnessed these new threats firsthand, they come to the PC's aid and escape the city. Being the only one to fight these threats firsthand, Cassandra takes you to meet the head of the Seekers (likely Leliana). Leliana/Other Big Boss realizes this is a new threat which will require the unification of all of Thedas to beat, and says this can't be something that the Seekers as part of the Chantry can handle.
She then declares the Inquisition formed and you, as the resident expert on these new enemies, are joined into their ranks, with Cassandra to accompany you. As events progress, you raise the prestige and reputation of the Inquisition until, tragically, Leliana/Other Big Boss dies, then appointing you as the new leader of the Inquisition.
Boom goes the dynamite.

The veil tears and your character has to fight a load of demons and shizz but manages to survive, Is rescued by Flameth (or someone) and told that only the power of the inquisition is enough to save Thedas. Maybe you are given some help or something but you are told to recruit Cassandra and Viv to help you do this.
Somehow people know that you were there when the veil was torn and that you survived. this means you should be the one to combat this because of your experience.

If you are Dwarf, It's a drinking contest.
If you are Human, It's an electoral debate.
Elves don't actually get to be the inquisitor, if you choose Elf, you will instead be the Inquisitors butler. All decisions will be made for you and the game will consist of cooking fine meals, cleaning house, driving your human master around in a horse drawn carriage, and taking out the garbage.
The final battle is against a particularly persistent stain on a fine Antivan rug. I'd recommend you come at it using the tactical menu as it's highly resistant to most modern cleaning supplies.

The door smashes open, then a hooded, dark figure enters the room and stabs the map on the centre table with the murder knife.
With a cold and terrifying voice he breaks the silence by saying:
"I'm taking over..."

Cue the Dragon Age logo!

This seems like the most likely scenario and similar to how Shepherd became a Spectre but with one key difference once they name us the Inquisitor and sic us on the one who tore the veil and tell us to stop them by any means necessary, no group has any authority over us so no one can stop us since we are an independent group formed away from the other factions but some groups might have an interest in what happens join your cause.

There is no progression, we see the events that cause the veil being torn and survive it for some reason some stuff happens, we reach wherever and then get named the Inquisitor which all happens within the 1st hour or so.

Yes but due to the massive information dump recently not sure exactly what is left, is there something specific you are asking about.

To be perfectly honest this sounds like the most reasonable explaination behind the creation of the organization.
I mean the more I think about it the more silly it is for any nation/organitaions to hand over any sort of power to a long dead group. Hell they hardly want to give any support to the Wardens until a blight hits. So it would have to be the Inquisitor making some of dramatic demand and can't be someone else saying 'We need you to lead'.

1) Doing something remarkable that shows leadership qualities/makes him/her a hero
2) Having connections/resources/manpower to back it up. That is, being a man or woman or significant power.
Or by, for lesser reasons, being a compromise candidate for leadership that turns out to excel at it.

You are assuming anyone in Thedas just hands over power to the Inquistion. Last I check, we will be focing people to acknowledge the authority of the Inquistion, one way or another.
Somehow we will aquire an army (prehaps the Seekers join early on which would explain Cassandra's presence) and from there its up to us, the player, to decide how and where to expand the Inquistion's influence.
Since the player character is present when the viel rips open, he may be seen by others as the one with the best understand of whats going on. Or prehaps its only the player who has the will to doing something about it. Thedas is in quite a mess by the time the game starts.

Perhaps in the early portion of the game, we stumble across an abandoned/overrun castle and take it for our own? Start telling people it's safe there, so they start coming over and bringing their skills with them. Merchants surviving raids, survivors from battles, that kind of thing.

The Divine is telling that everything will be fine , she has gathered high ranked seeker , mages , nobles , merchant who are willing to made a peace treaty .( guess you're one of them? ...)
Of course then the sky tear open , everybody dies.
Flemeth saves you.
You're all "we're all doomed now , doomed!/cry" .She , then , tell you about the old Inquisition (yeah because it was 1000 years ago , she's the only one remembering about it , with Brother Genitivi).
Anyway you go to the chantry to inform people of the Divine Death.
People already got the news and a new "less friendly" divine is about to take control.
You said , a dragon named Flemeth saved you.Cassandra , Varric , and Leliana are suddenly very interested in you "Just like the Warden and The Champion"...
You said you need a hobby so you're going to start an Inquisition ( sounds like fun!) , and investigate about the breach in the sky , the divine's death , etc...
Cassandra and Leliana lost their high rank in the chantry and wonder if it was an inside job.
So they follow you around to solve all this mess.
I do like speculations.^^
