I love any kind of quests, as long as I feel I have enough variety considering a playthrough on the overall. So having just fetching quests or just lore quests would be boring in the long run.
I agree that the trigger for Codex quests should be more sophisticated. I spent quite an amount of time in various RPGs doing "Codex" quests without a single idea of what I was doing and why, I just grabbed XP and didn't really care. In a nutshell, it would be great if those quests were more like adventure game quests, they would require for you to think a bit more, to find clues or maybe to solve a narrative puzzle.
I tend to skip Codex in the course of my real-time PT, and then I go back to it when I have a bit more time. I mainly feel that not reading it is not that damaging to my experience. If I felt it was necessary, I might do that more. For instance, you could have quests where you'd have to make a decision, and only reading Codex would help you to make the "correct" decision. I know there are a few instances of those in DA, but they're mostly simple side quests and decisions, like "touch stuff and free foe" or "leave stuff and gain XP".

indeed
and i also like quests that give a really special reward (like warden's keep - yes, that was DLC, but still!) - not just some trinket or a little money (not enough to buy even a half decent piece of equipment)

And, please, for the Maker's Sake, no babysitting quests - it's ok if the NPC has the sense to run and hide which BW is usually good at, but I hate having to keep someone alive besides PC and Co. I'm not exactly sure why but it makes me want to tear my hair out.
One thing I liked about 'Shepherding Wolves' was that Ketojan could actually back Hawke up - in spectacular fashion, too!

I liked the little fetch quests, where you'd find a random junk in DAII and turn it in to the person, sometimes I wanted more info about that
