If we happen to visit Orzamar in Inquisition, will our choice of king make a big difference? Like if we made Bhelen king, there's no more caste system?

It wouldn't go that far, IIRC Bhelen only loosened some caste restrictions to benefit Orzammar.
If it happened there would probably only be some dialogue changes unless you got to meet them in person.
