Does everyone on the team NEED to carry some of the same items? Example, the Combat Scanner (which by the way), is the Combat Scanner the same as the Combat Sensor?

Do they need? No.
You should equip yourself and your party according to the enemies you are fighting. For example, polonium rounds are quite useful when fighting Krogan, as it prevents them from regenerating.
Combat scanner (lvl 4-7) is an improved version of combat sensor (lvl 1-3). There is also another improvement for that particular equipment (combat optics, lvl 8-10).

Thanks caradoc. Here is a follow-up question (actually, two). Does each member of the team need to be equipped with, in this case, a scanner or just Shepherd? If I can save to slot for something more useful to each member, that would be great.

As long as at least one member of the squad has a scanner attached to the currently-equipped weapon, you can pierce the enemy's radar (provided the scanner is strong enough to do so).Sasnak Kid wrote...

Personally, I would never equip a scanner except perhaps for a Combat Optics (high-level scanner) on mayber a sniper rifle on a Shepard that has a sniper rifle talent tree. There are generally better weapon mods.
Note that equipped scanners have no effect when you are driving in the Mako since no weapon is considered unholstered.

Also note that starting @ item level 7, your weapons and armor will get an additional upgrade slot. Such equipment will start to spawn around character level 35.
