 During the Cerberus Side Quest, Admiral Kahoku's body failed to spawn at the last base, is there anyway to force it to spawn, or at least trigger the next side quest; Hades Dog?  I've heard of this happening to other people, you'd think BioWare would have fixed it by now, but I guess you really can't expect them to actually support their game... <_<

I've seen a number of people have this problem, but I have yet to see the cause diagnosed, and I'm afraid I don't know of a fix.
