Squad mates dying during suicide mission on second play through. This hasn't happened before.

I bought all the ship upgrades
Gained loyalty of my crew including the two DLC characters Zaeed and Kasumi
I thought it was weird when Kasumi died after doing the ventilation part since I figured she was a tech specialist. I went back and let Legion do it since I chose him during my first playthrough and he survived. But he died the exact same way as Kasumi. I wondered if it was because I chose Zaeed to lead the fire team but I just rage quit at this point. Can anyone shed some light to my dilemma?

My first play thru back in 2010 I did it blind and chose Samara. It did not work out for Thane in the vents, of course Thane will always die in the vents, but he seemed the natural choice to me as my infiltrator. You'd think Samara would have the skills to lead the fire team but I guess most of her career has been solo. Miranda complments you on the choice of Samara too btw.

Since that is not possible (Mordin is not good for anything except escorting the crew if loyal), I'll assume that you're mistaken. The only other way that would be possible would be if you hacked the game.shingara wrote...

To clear up any confusion: Tech Specialist - Tali, Legion, Kasumi
Team Leader - Garrus, Miranda, Jacob
Biotic Specialist - Jack or Samara
Escort - Any loyal
"Hold the Line" - Best: Garrus, Grunt, Zaeed. Good: Jacob, Thane, Miranda, Samara, Legion. Bad: Jack, Mordin, Kasumi, Tali
And it isn't always the specialist that will die if you pick the wrong one, sometimes it will be someone else, such as one of your picked squadmates if you pick the wrong biotic specialist. Also remember that any squadmates chosen for a role must be loyal to succeed in that role (except Miranda in the second fire team leader role).

I didn't know all your team mates could make it. So, since I started with ME3 and knew who I hadn't seen there, I started picking people based on their absence from a non-import ME3.
This meant Thane for the vents. Can't remember who I picked for for fire team leader. Thane died. My suspicions seemed confirmed: the game is going to kill whoever I pick.
Next up: pick a biotic specialist, escort, and second team lead. Well, I didn't see Samara in ME3, so she's the biotic. Can't remember who I picked for escort. (Everyone was loyal, btw.) For second team lead I picked Grunt since, again, I had not seen him in ME3. When we came out the other end Grunt died but everyone else made it out alive, much to my surprise.
Then I picked my final assault team. I don't remember exactly who I picked. I know I had Legion along because I wanted his drone. (I was new and used to playing an engineer in ME3 and missed my drone playing a soldier in ME2.) Anyway, Garrus stayed behind which was enough to save the rest of the team. (I did not have Zaeed or Kasumi yet. Got the DLC later.)
So despite thinking the game was going to kill just about everyone, turned out the only two I lost were Thane and Grunt.
Well, that's not quite true. I also lost Kelly and Gabby. After the kidnapping I still had not activated Grunt. Everyone was against waking him up, so I left him in his tank. But now that it looked like the game was almost over (I didn't know you could keep playing) I still had one item left in my journal: activate the Krogan. So I did and he immediately gave me his loyalty mission. So, again, thinking the game was almost over, I decided to go do his loyalty mission before launching the suicide mission. I thought this would be like other open world games where you put off quests for as long as you like. I didn't think there was any urgency to launch the mission. So of course, I got to watch as Kelly was turned into goo.
The next time I played I went and read the wiki first.

well i did have some mods installed so that could have altered things, didnt activly cheat mind you. thanks for that info.

I actually went in blind on my first playthrough, and I was kind of surprised how the game, more than once, forced Shepard to go on a priority mission, and this pattern kind of hinted to me how I should play. After Horizon and the Collector ship, suddenly the next priority mission was optional, and with Miranda's suggestion to continue building the team, I saw that as yet another hint that I should hold off on the IFF mission for as long as possible, so I did.
As for the vent, I figured why not throw Legion in there. It's a robot; it can handle it.
As for Grunt, the crew's disapproval of activating him actually made me more eager to do so, same for Legion.

Just to clarify - it doesn't matter who you send to escort the crew. It's just a better idea to send someone squishy (Mordin, Tali, Kasumi, etc.) who might die easily during the final defense. And as long as they're loyal, they'll live through the escort.RZIBARA wrote...

Yep this, exactly. Though as just said, you can send any LOYAL squadmate to Escort, and if you have enough of the invisible 'defense score', it doesn't matter who you send - Mordin is just the safest choice as he's the squishiest to be killed by a bad defense score. Garrus, Grunt, and Zaeed are just great defaults for a final defense, but not necessarily required.
Or something like that.
Taking non-Loyal squadmates on the final boss fight means they die. You need at least one character Loyal and on your final boss team in order to keep Shepard alive, I believe - meaning no Loyalty Missions = death.
Erm not much else.
It's sad how some squadmates have no role in my main Shepard's story
