I've been thinking about this a lot playing Volus. I kinda think of myself as a 3 shot kill. That graph seemed to justify Fitness 3 to me at least. Can take an easy mook bullet pre-gate. If I hit boost quick enough I can pump in the 50% DR bonus too. I'd say Fitness 3 is probably the Volus sweet spot if you're not packing Cyclos.

The average player, let's say who can clutch a silver wave and can deal with gold if the team is good, without fitness will not be able to stay alive without cyclonics, and will not have enough cyclonics to be able to play much.

disciple, leland gaunt, cassandra saturn, shards7(james brown), stratoblip, crash ur stox (we never duo but hes willin and hes excellent player), ferocious panda, and caineghis.
i dont spew "1v1" for the sake of epeen... i spew 1v1 out for the sake of actual ingame "theory" testing. the best way to find out what is better than what is for a duo on hard platinum settings..
ofcourse no fitness can look good on full team lobbys or scrub gold.

I'd only add that it's worth speccing into on any character that's gonna take fire no matter what you do, not only on tanks, i.e. vanguards and others. Also kits without a dodge.

If you're so sure damage output always trumps health and shields then why do you use Cyclonics so much? Shouldn't you use Power amps or even power recharge or stability amps to slightly increase your damage output?
I spec at least a little into fitness on most of my kits because I don't want to rely heavily on Cyclonics, I prefer using Power, Adrenaline and Shield Recharge amps.

I would if I had a more positive outlook on sniping in this game. Seeing as how I don't zoom in right where I'm initially pointing 95% of the time... I'm not too enthusiastic about it.
Besides, I have respec cards.

Some kits have no use for fitness, such as volus, many infiltrators and N7 demolisher for example, but that hardly represents every kit build in the game.
Kit had a power that gives damage reduction? Fitness gives a bigger bang for your buck when the DR's effectively acting as a shield multiplier by dividing the damage your kit takes by a percentage.
Kit has a power that you think is of limited use, such as decoy, submission net or lift? Might as well put 0,3 or maybe 4 points in the lame power then put the remainder in fitness.
Kit relies on biotic detonations for dps and doesn't bother with big guns? 66606 works just fine.
Kit lacks a dodge? Then your gonna get shot a fair amount, that's why the kit has high base shields. Fitness gives a better reward on investment when you have 750 base shields or better, since even the first three ranks will get you past the 1000 shields breakpoint where a lot of gold attacks won't shieldgate you.
Then there's the melee kits.
Then there's the option of using the armour slot for somthing else like a Power Amplifier Module if you aren't dependant on having a cyclonic to stay alive.
Then there's the issue of keeping a steady supply of cyclonic modulators.
Then there's the issue of what difficulties you play on:
Players who play nothing but platinum are a rarity, most players play one difficulty for when playing seriously, and a second, lower difficulty when relaxing. Others play a higher difficulty only with friends, and pug at a lower difficulty.
A few points in Fitness makes life in lolsilver a lot smoother when trying to chill in a quick game without using any consumeables, putting a few points into the first three ranks for this purpose is fine.
You mentioned several of these in the OP, but still, thats a lot of exceptions to the "fitness sux durr" rules.
