It's clearly possible without completing the game twice, or with an ME1 import, because I just got it.
Also this was my first playtrough that I'm certain of, and I never even played the first Mass Effect.

Anyone know what triggered this?

All possibilities I can think of is either it's a glitch or ME3 somehow made it happen?

With an import, yes. But the OP says he hasn't played ME1.

I imagine that is it.  Genesis creates an ME1 import, essentially.
