So, I was reading new codex entries and noticed that both my entries for Connor and Isolde say that they died despite the fact that I used the Circle to kill the demon. Is this a common glitch?

Variations of it seem to be. My codex always says Isolde sacrificed herself to save her son, even when I saved both of them.

Same here--I saved both people, and the codex said that Isolde sacrificed herself.
The good thing is, that didn't seem to have any effect on the game itself (how people reacted to my decision).

Yep, it's a good thing it doesn't have any effect, I just wish it was a proper update. One other thing I noticed in my first playthrough was that my journal said I didn't kill one of the generals (I think the one in the market) even though I got the XP for killing him. Ah well, maybe a fix in another patch?
