Nah, I didn't mess up like that.  I did the mature thing and walked away from her BS at the end.  I still hit every charm line, so I would of made her sound like a lunatic anyways.  I just like walking away at the very end, instead of wrapping it up.  I've done the making her sound crazy before though.

Maybe if you chose the Charm option from ME 1, she got fired and she came for revenge and tries to stab you with an AIDS needle.

I don't think that choosing the top choice will make you punch her, to me it looks like it later in the conversation when you punch her

I didn´t even know you could punch the reporter lady till I saw it on youtube today oO. But it doesn´t have this falcon pawnch look in ME2, would have been funnier to just give her the slapsgiving of her lifetime.

If in ME1 you went with pro-human (= renegade) answers she is quite satisfied.
If you went with paragon answers, then she is really pissed off and Hacket later tells you that she made you look like a freak in her show. So the paragons have reasons to punch her this time 'round, and the renegades (generally) don't.

Well remember when you can kill finch without any consequences for just lying to you...The turian even gives you credits for it. And saren betrayed the council by attacking eden prime and killing nihilus.
