What is the hit cap that makes you hit 100%? I have perfect striking, but is is needed??

It depends on the enemy. You need 46 more attack than their defence to have a 100% chance. White mobs will have around 80 defence at high levels, some bosses and elite bosses can get up to ~110 (gaxkang has like 120 in revenant form as well as a crapton of armor)
