I'm not sure if I'm remembering this correctly, but I recall seeing a video where classes with Charge can move through physical objects to their target, basically transporting themselves. Does this mean that there are normally unreachable enemy locations (and maybe secret items) accessible only to characters that can Charge?

Shouldn't be.
You'd still have to be able to target them.
Most targetable enemies are walkable.
And if it's true, it seems a tad unfair to non-Vanguards.
All in all, I doubt it.

I remember a comment that you can't charge to a location you can't otherwise walk to. So no.

Probably not, the walls will still have invisible barriers that prevent you from going outside the map.
