Ok, so I am in the Lower Ruins and I have encountered a fountain and an Alter. It would appear that I need to do something with these two things to get the door to open which should lead me to the Werewolves Lair. I have tried everything that I could think of, so I reached out on the web and found this -
1. Fill the jug with water and take it >>> done
2. Place it on the altar >>> done
3. Kneel and pray >>> done [afterwhich we are attacked, we defeat the ghostly enemies]
4. Take a sip from the jug >>> no option to take sip, the following options are the options available and their result -
examine the carvings on the alter more closely >>> cycles back thru options that has no impact
examine earthen jug on the alter >>> take jug or ignore the jug
leave an offering on the alter >>>senses this is wrong, coins returned
kneel before the alter and pray >>> nothing happens
leave the alter alone
5. Pour the rest of the water back into the pond >>> jug is empty
The only option to take a sip is if you choose to examine the earthen jug on the alter. However, if you take a sip, then kneel and pray, nothing happens. If you take the jug, as I stated earlier, it is empty. So, as you can see, I am stuck.

Earlier on in this level there was a sarcophogus in a side room which contained a clay tablet of instructions for what to do at this altar. If you do not have the tablet, then the shades will appear and fight, as they did for you (this should not happen if you are doing the ritual with the tablet) and the ritual will not work.
So either retrace your steps and search every side room until you find the tablet (it is located in the Lower Ruins, so do not go back up to the previous level), or you can skip this quest if you like. It is only a side quest and not necessary to complete the main mission.

And yes, you do need the tablet.

You do know that there is a (platform-dependent) key that will highlight all interactable items in your vicinity? I use it all the time to make sure I don't miss anything. As I recall it is TAB for PC.

I play on Xbox myself, and there it is the left trigger (the same that brings up the power wheel). It shows the labels for all interactable objects in the area. It is great for finding all the barrels and elfroots that can be tricky to spot otherwise. I really missed this feature in Skyrim.
