Point 1: Mharen's phylactery stops cold at the foundry
Point 2: Ninette's wedding ring is found on her severed hand. In the foundry.
Point 3: Said foundry had a man running away upon noticing Hawke. Hawke also saw this man (diplomatic dialogue with Emeric at quest's conclusion)
Point 4: Long after Quentin fled the scene, and only after Hawke enters the place, Demons show up. Further, some companions note that these were likely the result of a blood mage (Fenris in particular).
Point 5: Both Ninette and Mharen were given lilies.
All of this tells me that the place should've been investigated and thoroughly guarded because of the simple fact that demons were present.
Demons were present. Yet Aveline decided "Well, Kirkwall's not a safe place, so LOLOLOLOL" and Meredith just brushed aside everything thinking it wasn't their place to investigate.
Yeah right.
If the demons weren't present, I could buy both Meredith's and Aveline's opinions on the matter. And of course, you have to be pretty incompetent to not find a hidden chamber door.
Regarding Orsino, since the note in question is almost certainly from before Leandra's death (and thus he had no way of knowing she would be a victim) and talks about "Quentin proving Orsino wrong again", I believe that Orsino was led to believe from Quentin that the latter was researching ways into true resurrection via blood magic.
Quentin's motive was bringing his deceased wife back to life. His MO was not going to work in that regard, but I think since he was off his rocker he wasn't giving Orsino enough details for the FE to fully grasp how he was going about it,
until Quentin sent him "LOL look at this cool thing I made with corpses!" research.
Then Orsino goes "****" and puts the research aside.
And then the whole "ONE MORE GAMEPLAY BOSS" contrived bull****.
