What is Thane referring to when he says, "And should the worst come to pass, grant me forgiveness."
Is he speaking of the possibility of him dying?

Don't know. Has puzzled me a bit too. Possibly killing someone other than the target? Dying?

I believe it means if he has to kill an innocent. He is, after all, a very religious man. However, having to kill innoncent people isn't going to stop him from completing his mission. His body is train to be an assassin but his soul will regret the sin. Drell have that werid and very literal view of the duality of the Soul/Body. He also prays after killing each of his targets to ask forgiveness not just for them but for himself. They might have deserved dieing but murder is still a sin.
