(like Malcolm Hawke's friend, or the naive french guy who wanted to find "love"...) You just have to want to look.

You need to keep in mind that a series of games like this, with its own books, comics and various other tie-ins, requires a LOT of work, that can last years. Also, there is not a single writer at work on the series, but several, and often things that were extablished at the beginnig are changed in the course of work - example: Alistair was initially supposed to be much older.
Even with constant quality control mistakes can happen and consistency be broken. There are several examples of this - like dates being messed up in The Calling. It's just a human thing and we need to be a bit flexible about this.

There should have been an option to hand Merrill over to the clan after the Keeper's death (when I played DA2 I was fully expecting the option to appear).

It's either:

All three of these can work with competant writing. Which Dragon Age 2 lacked. The thing that explains Kirkwall's corruption (and probably one of the most major things that could explain the insanity mages keep going into) is in three codex entries that you have to scour the game for.
Orsino also just goes "insane" for literally no logical reason in the mage ending. And even Bioware admits they did it because "they needed another boss" that is bad writing
They 'tried' to do your third point but fumbled it so bad I just stopped caring the entire conflict altogether because everyone is going insane, incompetant, or Anders.

He or she is still the antagonist by virtue of being in opposition to the player character.
