Any time a player model falls into the void (the area beneath the physical boundaries of the map) they will be respawned into the map. Most maps have the LZ as their spawn point. The reason for this is that the void is infinite, and completely devoid of physical objects. If an object falls into the void it will continue to fall potentially forever unless it is deleted. To prevent players from getting stuck falling forever, falling a specific distance below the and touching the invisible failsafe will delete your character and respawn them at the LZ. This is not actually a glitch, but a continence in the event of an actual glitch, being usually clipping errors.
These commonly occur after being sync-killed because the player model is relocated after a sync-kill. If you are synced in an awkward location, the game may place you inside of an object. The physics engine will attempt to push you out of the object, usually down through the floor. Most maps lack true overhangs and will drop you in the void.
Another common way this happens is that the invisible barriers that surround the map do not affect ragdolls. For example, the ledge directly behind the spawn on giant has an invisible wall that prevents players from simply walking off the edge. If you are downed in that spot however, your body can simply fall right off the edge unimpeded.

More annoying is the stealth synch kill which I've seen twice. I've had a narrow escape from a situation where I should have been synched, once with a banshee once with a praetorian. I'm able to run around and shoot for a bit whilst the enemy jusy stands around, then I drop dead, which is just rude. If it happens again I'll have to try and missile the enemy that caused it, but both times before I didn't realise I'd actually been picked up, until death happened...
